Post Majority Nurse-Outreach position provides a combination of duties to ensure that the youth/adult has health and medical support in place to support their health and well-being. The Nurse is responsible for determining on-going health and medical supports of the youth/adults to determine what is required for their health and well-being. Generally, performs on-going physical assessments of youth/adults along with consultation between the Caseworker, Elders, Mental Health Therapist, and family as well as other significant persons involved in youth/adults’ life. Work with youth/adults to address their health issues to improve their physical health within a trauma informed framework and integrating a client centered and strength-based approach. The Post Majority Nurse is to work in collaboration with Elders to ensure that youth/adults have access to Siksika culture and traditions while on their healing journey.
